Hospice provides a coordinated program of palliative care to terminally ill patients, and supportive services to patients, their families and significant others, in both home and facility settings, through a physician-directed plan of care. Specialized services include medical, nursing, home health aide, social services, spiritual care, bereavement services, volunteer services, and other appropriate counseling services.
The hospice philosophy of care provides support for persons in the last phases of incurable illness so that they may live as fully and comfortably as possible. Hospice care seeks to enable patients to lead an alert, pain-free life and to manage symptoms so that their last days may be spent at home or in a home-like setting.
At Essential Hospice, our caregivers are available seven days a week and our medical team is available 24 hours a day, seven days a week.
Routine Home Care
Routine Home Care is for individuals receiving hospice care wherever your home is located, whether it is your home, an assisted living facility, a personal care home or a nursing home.
Inpatient Respite Care
Inpatient Respite Care is for individuals in an inpatient facility who need care up to five days at a time.
Continuous Home Care
Continuous Home Care is for individuals that need services for at least eight hours within a 24 hour period for brief periods of crisis and only as necessary to maintain the terminally ill individual at home.
General Inpatient Care
General Inpatient Care is for individuals in need of hospice in a Medicare-certified hospital, skilled nursing facility, or inpatient unit of a hospice.
